Special Passports

There are now various Passports for a range of specific purposes. Not everyone would necessarily use the term 'Passport' for one of the following documents. But whatever 'name' is used, the principle is the same, i.e. important information distilled into a simple and easily accessible format.

Mealtime Passport Placemats

Angela Crocker, a speech and language therapist in Belfast, pioneered Personalised Place Mats as a simple and effective way of summarising the information that can help adults with learning disabilities to positive, safe and successful mealtimes.

This is a form of 'Specialised Passport' or 'Single Focus Passport', in this case for eating and drinking (This style could also be used for topics such as IT access; behaviour support; using my vision; my medication; or topics where photos are needed to permit correct positioning and/or equipment use).

National Autistic Society (NAS) My Health Passport

The NAS My Health Passport was developed by Baroness Angela Browning, an NAS Vice President, in collaboration with the National Autistic Society.

'My Health Passport' - a resource for autistic people who might need hospital treatment.

A steering group within Kent and Medway NHS Trust, consisting of a multidisciplinary group of professionals, reviewed the 'My Health Passport' in 2017 and updated the passport following comments by autistic people and professionals about how they were using it or wanted to use it.

Widgit Health: My Health Passport

This was developed by Hywel Dda Children's Services, Sister Donna Richards, WellChild and Janet Millward Senior Nurse, Paediatrics. The Widgit Health 'My Health Passport' uses Widgit symbols and is available in English and Welsh with a range of other resources available in different languages.

TD Snap Medical Passport and Pain Scale

The Medical Passport and Pain Scale pagesets can be downloaded to the TD Snap app. There is a customisable one-page medical passport and a detailed pain rating scale. They can be found on Pageset Central and are by a creator named Dani.

How to download and add the pagesets:

  1. Click the link
  2. Log in to your my Tobii Dynavox account
  3. Add to 'My Stuff'
  4. On your TD Snap app, select a blank button
  5. Import and link to page
  6. Select 'Medical Passport Template' then 'import'
  7. Select another blank button, import and link to page and this time, select 'Pain Scale'
