Gathering Information

The information included in a Communication Passport should be what the individual wants to share and what matters to them. This should be filled in collaboratively with the individual/owner of the passport, family, education staff and any other relevant people who know them well e.g. therapists. One person or a few of these people should make up the core group responsible for producing and maintaining the Passport.

Communication Passports as a People-Connecting Profile

The information included in a Communication Passport is to aid conversations, connect with an individual and build relationships. It is not solely for sharing information between professionals.

We want to involve the individual as much as they are able in the creation of their Passport. Conversations regarding the content and style are essential.

To help guide conversations and gather information to include in the Passport, you can use this editable 'Information Gathering Document'.

It might be easier in some cases, to share a digital form. The links below will take you to a Google Form Template. Copy, rename and share the link with whomever you are collecting information from. It is set by default as 'Restricted Access' permissions, meaning the recipient would be prompted to request access and the person who has shared the form would then receive a notification to grant access.
